If your student has been accepted at Summit Sierra, congratulations! If you have not done so already, please complete your registration by bringing the following documents to Summit Sierra:
- Proof of legal name and birthdate, such as a birth certificate, passport, baptismal certificate, etc.
- Certificate of Immunization Status or immunization records. We are required by the state of Washington to collect certain immunization data about all students. An immunization exemption form is available upon request.
- Proof of address, such as a utility bill, rental agreement, property title, or driver's license with current address.
